The FP10 Team is responsible for the ordering, storage, supplying and monitoring of FP10 pads for Liverpool community physical health, Liverpool community mental health, Southport and Formby, South Sefton non medical prescribers (NMPs) and Mid Mersey.

Ordering:

All NMPs requiring a supply of prescription pads must email attaching a prescription request form to FP10Requests@merseycare.nhs.uk giving five working days notice. 

Collections:

FP10 pads can be collected from Medicines Management Building, subject to an email appointment. When collecting, please have your Trust badge ready and appointment number which you will recieve when your appointment is confirmed. If you are collecting on behalf of a colleague, they will need to send an email to FP10Requests@merseycare.nhs.uk to inform the team.
